CONTRACT 2999K AmendmentAgreement No. 2999K ELEVENTH AMENDMENT TO AGREEMENT NO. 2999 BETWEEN THE CITY OF EL SEGUNDO AND GARY V. BUFKIN dba GARY V. BUFKIN COMPUTER CONSULTING THIS ELEVENTH AMENDMENT ("Amendment") is made and entered into this lst day of October, 2017, by and between CITY OF EL SEGUNDO, a general law city and municipal corporation existing under the laws of California ("City"), and GARY V. BUFKIN dba GARY V. BUFKIN COMPUTER CONSULTING ("GVBCC"). The parties agree as follows: Pursuant to Section 15 of Agreement No. 2999, executed April 16, 2002, as amended, ("Agreement"), Section 1(A) of the Agreement is amended to add the scope of services set forth in attached Exhibit "A", to this Amendment. In the event of a conflict between the Scope of Services in the Agreement and the Scope of Services attached as Exhibit "A," the Scope of Services in the Agreement will control. 2. For such additional services, the City agrees to pay an additional, not to exceed amount of $10,000. 3. Section 2 of the Agreement is amended to extend the term to September 30, 2018. 4. This Amendment may be executed in any number of counterparts, each of which will be an original, but all of which together constitutes one instrument executed on the same date. 5. Except as modified by this Amendment, all other terms and conditions of Agreement will remain the same. Hensley, City Attorney 1,�-<,� '1--- 2 Agreement No. 2999K October 1, 2017 Fiscal Year 2017/2018- Exhibit "A" Service Agreement Gary V Bufkin Computer Consulting provides computer programming and maintenance services for the City's multi -departmental system (Muni Permits). The system currently allows Plan Check tracking and permit issuance for Building Safety, Planning, Fire Prevention, Public Works, Police, Code Enforcement, and Recreation & Parks. Muni Permits allows staff to issue permits and track a development project from beginning to end. The software is warrantied for as long as you use it. If a bug or problem is encountered which is the responsibility of GVBCC, it will be fixed and maintained at no charge to the client. Description of Services Consultant shall provide continuous maintenance support and warranty for all Online Permits Center (OPC). Muni Permits System Software and Project Builder in place at no charge to the city for bug fixes, items installed and/or previously billed. Any addition, alteration or change to Muni Permits System Software that exceeds $4,000.00 shall be made by Purchase Order that includes specific scope of services and estimated time and cost. Each Purchase Order shall require the approval of the Information Systems Manager/Department or Building Division Manager. Paymen GVBCC shall present invoices to the city for payment on a quarterly basis (4/1/18, 7/1/18 or 9/30/18) and at completion of purchase order (whichever comes first). Invoices received more than 30 days after completion of Purchase order or end of quarter are subject to extended and delayed time for payment. Time for Com letion September 30, 2018 Hourly rate for additional services $125/Hour *Total amount of contract and Purchase Orders not to exceed ten thousand dollars ($10,000).
