CONTRACT 4940B CLOSEDAgreement No. 4940B SECOND AMENDMENT TO AGREEMENT NO. 4940 BETWEEN THE CITY OF EL SEGUNDO AND GALE JORDAN ASSOCIATES, INC. THIS SECOND AMENDMENT ( "Amendment ") is made and entered into this 10th day of March, 2016, by and between CITY OF EL SEGUNDO, a general law city and municipal corporation existing under the laws of California ( "CITY "), and GALE JORDAN ASSOCIATES, INC., a California Corporation ( "CONSULTANT "). 1, Pursuant to Section 35 of Agreement No. 4940 ( "Agreement "), the term of the Agreement described in Section 8 is extended to December 31, 2016. 2. In addition to the amount set forth in Section 1(C) of the Agreement, the CITY agrees to pay CONSULTANT an additional sum not to exceed one thousand, three hundred twenty dollars ($1,320.00) for CONSULTANT'S services, at the rates specified in the attached Exhibit "C," which is incorporated by reference. 1 Exhibit C includes the attached cost proposals for the following locations and activities: A. Recreation Park Facility - Gordon Clubhouse — testing paint on the exterior walls /stucco of the building. 4. This Amendment may be executed in any number of counterparts, each of which will be an original, but all of which together constitutes one instrument executed on the same date. 5. Except as modified by this Amendment, all other terms and conditions of Agreement No. 4940 will remain the same. Gale Jordan Associates., linc, Taxpayer IlD No. 33-0322878 Agreement No. 4940B EXHIBIT C SCOPE OF ADDITIONAL WORK FOR SECOND AMENDMENT TO CONTRACT NO. 4940 P ojeet..S eciljc Sco_l�e The Consultant, Gale Jordan Associates, shall provide additional sampling, testing and assessment services with the ultimate goal of removing any harmful substances, such as lead and asbestos, at the following facility, located in the City of El Segundo, CA 90245: 1. Recreation Park Facility — Gordon Clubhouse — testing paint on the exterior walls /stucco of the building. • Sampling can occur as soon as possible after the agreement is amended and all signatures have been obtained. • Each report shall identify any lead, asbestos, and any other harmful substances on the subject building. These additional projects will achieve the same following objectives: 1. Comprehensively test for any lead -based materials, asbestos or harmful substances inside the affected areas of each building. 2. All necessary mobilizations for the successful bidders to complete the project shall be included in the bid costs. Visits will be coordinated by City staff when a bid is awarded. 3. Generate a comprehensive report identifying all the findings of the tests a. The report shall identify the locations from where each sample was taken, show the material identified, show the amount of any potentially harmful substance present, and compare the existing amount to acceptable amounts of the substance. b. The report shall clearly identify what locations will require abatement if disturbed for removal and construction. c. The analysis in each report is to include recommendations for abatement of any potentially harmful substances during removal and construction. d. Each report shall provide abatement objectives and recommended methods of treatment /mitigation for Contractors and abatement monitoring companies for its subject building during removal and reconstruction. e. Each report will be made part of the specifications for its subject areas. • The Consultant will provide one electronic report for the Recreation and Park Facilities combined to the City which will contain a detailed implementation of all the project goals listed above. The Consultant shall be properly certified for the work proposed with all certifications up to date. General Scope conditions shall remain the same unless otherwise stated. (gja) is pleased to provide the following proposal for the above - referenced project site. The sampling will be limited to suspect asbestos - containing and lead- containing materials on the exterior of the Recreation and Parks facility, the Gordon Clubhouse. Asbestos /Lead Bulk Sampling The limited survey for asbestos - containing materials (ACM) will be performed in accordance with Cal /OSHA guidelines, which meet the requirements of the South Coast Air Quality Management District. The survey will be performed under the supervision of a Cal /OSHA Certified Asbestos Consultant. The asbestos samples will be analyzed by a third - party, independent laboratory using Polarized Light Microscopy with Dispersion Staining (PLM/DS) in accordance with the Environmental Protection Agency (EPA), "Method for the Determination of Asbestos in Bulk Building Materials" (EPA 600/R -93 -116). Percentage estimates of each material component are based on the analyst's best judgment following examination with a stereoscope and PLM/DS analysis. Bulk samples will be collected of suspect lead- containing materials in accordance with the Cal /OSHA requirements of 8 CCR 1532. 1, in order to evaluate potential worker exposure to lead paint. Bola March 8, 2016 Page Two Bulk samples will be collected of suspect lead - containing materials in accordance with the Cal /OSHA requirements of 8 CCR 1532. 1, in order to evaluate potential worker exposure to lead paint. The samples will be collected by scraping approximately one gram of paint onto a moistened surface, in accordance with the guidelines found in the Department of Housing and Urban Development's (HUD) "Guidelines for the Evaluation and Control of Lead - Based Paint Hazards in Housing ". Samples of ceramic tile will be collected by chipping off small areas of material. The samples will be analyzed by a third -party, independent laboratory using Flame Atomic Absorption Spectroscopy in accordance with EPA Method SW846 -7420. The surveys will be performed under the supervision of a CDPH Certified Lead Inspector /Assessor. The report will include laboratory analytical data, estimated quantities and condition assessments, and recommendations for future actions. Please note the survey will be limited to accessible areas. It is the responsibility of the client to ensure access areas that may require special equipment (e.g., lifts or ladders). The survey will be limited to visible and accessible materials. No destructive sampling will be conducted. Bola March 8, 2016 Page Three Confidentiality and Limitations This report will be based upon conditions and practices observed at the property and information made available to gja at the time of the site visit. gja's assessment of the risk of exposure to asbestos and lead will follow generally accepted protocols and will be based on conditions at the time of the survey. This report will not propose to identify all hazards or unsafe practices, or to indicate that other hazards or unsafe practices do not exist at the premises. Additional suspect but un- sampled materials could be located between walls, in voids, or in other inaccessible areas. The report will be prepared for the sole use of the Client, the only intended beneficiary of our work. gja understands that our services to the Client are to be held in strict confidence. gja will not discuss or disclose any information about our services to any third party without the Client's consent. gja is not responsible for changes in conditions or accepted protocols subsequent to our site visit.
