FOURTH AMENDMENT TO
AGREEMENT NO.39sm BETWEEN
THE CITY OF EL SEGUNDO AND
SCANNING SERVICE CORPORATION
THIS FOURTH AMENDMENT ( "Amendment ") is made and entered into this _1ST_ day
of October 2012, by and between CITY OF EL SEGUNDO, a general law city and municipal
corporation existing under the laws of California ( "City "), and Scanning Service Corporation, a
California Corporation ( "Consultant ").
1. Pursuant to Section 35 of Agreement No. 3987D ( "Agreement'), Section 8 of the
Agreement is amended to extend the term to September 30, 2013,
2. Pursuant to Section 35 of the Agreement, Section 1(C) of the Agreement is amended
to read as follows:
"As additional consideration, CITY agrees to pay CONSULANT a sum not to exceed
TWENTY FIVE THOUSAND dollars ($25,000) for CONSULTANT'S services for
contract total of $25,000 through September 30, 2013. CITY may modify this
amount as set forth below. Unless otherwise specified by written amendment to this
Agreement, CITY will pay this sum as specified in the attached Exhibit "A ", which is
incorporated by reference."
3. This Amendment may be executed in any number of counterparts, each of which will
be an original, but all of which together constitutes one instrument executed on the
same date.

4. Except as modified by this Amendment, all other terms and conditions of Agreement
No. 3987 will remain the same.

Scope of Services
Contractor (SSC) shall perform to the City, scanning of plans, permits and microfilm/fiche to
digital Image for the Questys Imaging System.
1. SSC shall pick up documents From the City of El Segundo (client).
2. Documents will be transported back to SSC's facility In Rancho Cucamonga, CA for
scanning.
3. Documents will be originally prepared for scanning.
4. Documents will be scanned. Every page will be viewed as scanned for quality control.
Every page will be adjusted for best Image.
6. Files will be set up in Questys format with corresponding data. Scanned pages will be
imported into these files.
6. Documents will be QCed for image quality and data accuracy.
7. Documents will then be put on CD for transfer back to the City along with the paper/film
files. (Unless client wants SSG to hold original flies for destruction at a later date). If SSC
holds the files, they will be held for 90 days to allow the City to verify images and data
and then they will be shredded and a Document of Destruction will be provided to the
City.
Contractor will use its' own transportation to pick up and deliver files. All documents will be held
"Confidential" while in our possession.
Contractor will provide an email copy of any file requested by the City while in SSC's possession
within 24 hours.
Contractor will work with the City to ensure the successful completion and Implementation of the
project.
Contractor will provide monthly invoices for work performed and delivered on CD to the City,
