'AMENDMENT TO
AGREEMENT NO. 3917 BETWEEN
THE CITY OF EL SEGUNDO AND
MAUREEN SASSOON
THIS 2ND AMENDMENT ( "Amendment ") is made and entered into this Ist day of October,
2010, by and between the CITY OF EL SEGUNDO, a general law city and municipal corporation
existing under the laws of California ( "CITY "), and MAUREEN SASSOON, a sole proprietor
( "CONSULTANT ").
1. Pursuant to Agreement No. 3917 ( "Agreement "), executed by the parties on or about
February 25, 2009 and subsequently amended, the term of the agreement is extended
until September 30, 2011.
2. This Amendment may be executed in any number of counterparts, each of which will be
an original, but all of which together constitute one instrument executed on the same
date.
3. Except as modified by this Amendment, all other terms and conditions of Agreement
No. 3917 remain the same.
4. Exhibits A, B, and C are incorporated by reference.
IN WITNESS WHEREOF the parties hereto have executed this contract the day and year
first hereinabove written.

EXHIBIT "A"
SCOPE OF SERVICES PROVIDED BY DR. MAUREEN SASSOON CIH:
Dr. Maureen Sassoon, CIH to continue providing professional Occupational and
Environmental Health & Safety Consulting Services in fiscal year 2010 -2011, effective
October 1, 2010 through September 30, 2011, for an amount not to exceed $25,000 will
include, but are not limited to, the following:
1. Conducting the City Safety Committee Meetings, pursuant to the City's Injury,
Illness Prevention Program, six times per year (includes written records from each
meeting).
2. Employee Health & Safety Training (i.e., Cal -OSHA mandated annual citywide
employee safety training (respiratory protection, hearing conservation, lockout -
tag out, hand tool safety, hazard communication, first responder awareness, blood
borne pathogens and emergency action /fire response, confined space, supervisor
safety inspections and accident investigations, office safety, employee driver
safety for class A and class B drivers and hands -on defensive driving course),
City -owned facilities and equipment safety inspections, and other safety training
on an as requested/needed basis.
3. Inspection services for Certified Asbestos Consulting, per DHS.
4. Inspection services for Lead - Certified Consulting, per DHS.
5. Annual City -Wide Safety Inspection.
6. Emergency CUPA backup for Environmental Safety Manager, on an as requested/
needed basis.
7. Dr. Sassoon to advise and represent the City's interests in issues involving Cal -
OSHA.
8. Other third party safety services on an as- needed/required basis: employee work
station and worksite ergonomic evaluations, Inspection services for based paint,
air quality, noise, and other types on an as- needed/required basis, as requested by
Risk Manager and/or Human Resources.

EXHIBIT `B"
FEE SCHEDULE:
The fees for training and meetings are based upon a $300 flat fee for the first two hours
and $$S for each additional hour.
Additional charges, such as laboratory fees for industrial hygiene monitoring (i.e.,
asbestos, indoor air quality, mold, and noise) are charged at the lab rate for analysis
requested.
