Dear Chief Smith:
We have reviewed the additional documents submitted by your department for the Continuing Education
Provider Program and are pleased to inform you that all program requirements as outlined in Reference No.
1013 - Prehospital Continuing Education Provider Approval and Program Requirements have been met. Re-
approval for the CE Program has been granted for an additional four (4) years and will expire October
31, 2006. Your California Prehospital Continuing Education Provider (CEP) number will remain 19 -0222.
You will be notified by mail six months prior to the expiration date regarding the need for re- approval of your
Continuing Education Program status. However, if the renewal information is not received, it will remain your
responsibility to submit a renewal application (Reference No. 1013.1) and supporting material sixty (60) days
prior to your expiration date. This will prevent a potential lapse in your ability to provide CE credit.
