FIRST AMENDMENT TO
AGREEMENT NO. 6910 BETWEEN
THE CITY OF EL SEGUNDO AND
PACIFIC HARBOR MEDICAL GROUP
THIS FIRST AMENDMENT ("Amendment") to Agreement No. 6910 ("Agreement") is
made and entered into this 12th day of September 2025, by and between the CITY OF EL
SEGUNDO, a general law city and municipal corporation existing under the laws of
California ("CITY"), and PACIFIC HARBOR MEDICAL GROUP, a CALIFORNIA
CORPORATION ("CONSULTANT'). The parties agree as follows:
1. Pursuant to Section 34 of the Agreement, Section 7 (term) of the Agreement is
modified to extend the term so that the Agreement will now expire June 30, 2026.
2. Subsection A of Section 2 is amended to include Exhibit " A," which is incorporated
by reference.
3. This Amendment may be executed in any number or counterparts, each of which
will be an original, but all of which together constitutes one instrument executed on
the same date. In accordance with Government Code §16.5, the parties agree that
this Amendment will be considered signed when the signature of a party is
delivered by electronic transmission. Such electronic signature will be treated in all
respects as having the same effect as an original signature. CONSULTANT
warrants that its signatory (or signatories, as applicable) to this Amendment has the
legal authority to enter this Amendment and bind CONSULTANT accordingly.
4. Except as modified by this Amendment, all other terms and conditions of the
Agreement remain the same.

As discussed in our cover letter the Pacific Harbor Medical Group has been providing medical
care to the community for over 20 years. The physicians, all of whom are board certified in
internal medicine or D.O., have had extensive experience providing primary care on both an
outpatient and in -patient basis.
Pacific Harbor Medical Group has provided a variety of medical services to various
municipalities and private industry including pre -employment medical exams, POST pre-
employment exams, executive physicals, annual physicals, DMV exams, return to
work/fitness for duty evaluations, DOT and non -DOT drug and alcohol testing, post -
accident evaluations, and vaccinations. All physicians are certified to perform DMV
Commercial Driver Medical Testing and are familiar with the disability laws related to
employer required medical exams. All of our physiologists are certified to perform DOT
Drug and Alcohol Testing.
B. Scheduling and facility
i. Scheduling needs: Appointments are available 7:30-5:00 Monday — Friday. We will work with
your staff to meet your needs.
ii. Description of our facility:
We occupy 5,000 square feet in a modern office building at the corner of Grand Avenue and
Pacific Coast Highway in El Segundo. The entire facility is handicap accessible and is in
compliance with all ADA requirements. Parkin is validated.
Our office is equipped with:
• A hospitable waiting room equipped with free Wi-Fi.
• Contemporary examination rooms.
• Physician consultation rooms.
• Vision/hearing /body fat analysis room.
• Laboratory with a dedicated bathroom.
• X-ray room
• Fitness room (with two digital treadmills and fitness testing equipment)

o Each patient will work one-on-one with an Exercise Physiologist or Medical Assistant throughout the testing
protocol. The Exercise Physiologists and Medical Assistants on staff are well trained in every aspect of the testing
protocol. This allows for a timely and comfortable transition through the physical. It also allows the Exercise
Physiologist and Medical Assistant to get to know the patient to help assist in developing an appropriate exercise
prescription and advise lifestyle modifications per conversations with that patient.
