THIRD AMENDMENT TO
AGREEMENT NO. 5856 BETWEEN
THE CITY OF EL SEGUNDO AND
OCCUPATIONAL HEALTH CENTER OF CALIFORNIA,
A MEDICAL CORPORATION D/B/A CONCENTRA, INC.
THIS THIRD AMENDMENT ("Amendment") is made and entered into this 22nd day of July, 2025,
by and between the CITY OF EL SEGUNDO, a general law city and municipal corporation existing
under the laws of California ("CITY"), and OCCUPATIONAL HEALTH CENTERS OF
CALIFORNIA, a California Medical Corporation d/b/a Concentra, Inc. ("CONSULTANT"). The
parties agree as follows:
1. Pursuant to Section 34 of Agreement No. 5856 ("Agreement"), as amended, not to exceed
amount descrived in Section 1.0 of the Agreement is amended to not to exceed fifty
thousand dollars ($50,000) for CONSULTANT'S services per fiscal year. The Parties
agree that this Amendment will take effect retroactively on July 1, 2025.
2. Pursuant to Section 34 of the agreement, Section 7 is modified to extend the term through
June 30, 2026.
3. Section 2.A of the agreement is amended to include the Attachment 1 to this amendment
and incorporate herein by reference.
4. This Amendment may be executed in any number or counterparts, each of which will be
an original, but all of which together constitutes one instrument executed on the same
date. In accordance with Government Code §16.5, the parties agree that this Amendment
will be considered signed when the signature of a party is delivered by electronic
transmission. Such electronic signature will be treated in all respects as having the same
effect as an original signature. CONSULTANT warrants that its signatory (or signatories,
as applicable) to this Amendment has the legal authority to enter this Amendment and
bind CONSULTANT accordingly.
5. Except as modified by this Amendment, all other terms and conditions of Agreement No.
5856 remain the same.
