CONTRACT 6747A AmendmentAgreement No. 6747A FIRST AMENDMENT TO AGREEMENT NO. 6747 BETWEEN THE CITY OF EL SEGUNDO AND ALLISON TRAINING PERSPECTIVES AND SERVICE THIS FIRST AMENDMENT ("Amendment') to Agreement No. 6747 ("Agreement") is made and entered into this 1 st day of July 2025, by and between the CITY OF EL SEGUNDO, a general law city and municipal corporation existing under the laws of California ("CITY"), and Allison Training Perspectives and Service, a Sole Proprietorship ("CONSULTANT"). The parties agree as follows: 1. Pursuant to Section 3 of the Agreement, Section 5 is modified to extend the term through June 30, 2027. 2. Pursuant to Section 3 of the Agreement, Section 18 is modified to establish a not to exceed compensation amount of $3,500 per fiscal year. 3. Pursuant to Section 3 of the Agreement, Section 20, Exhibits is modified to amend the scope of work to the services set forth in Attachment 1 hereto. 4. This Amendment may be executed in any number or counterparts, each of which will be an original, but all of which together constitutes one instrument executed on the same date. In accordance with Government Code §16.5, the parties agree that this Amendment will be considered signed when the signature of a party is delivered by electronic transmission. Such electronic signature will be treated in all respects as having the same effect as an original signature. CONSULTANT warrants that its signatory (or signatories, as applicable) to this Amendment has the legal authority to enter this Amendment and bind CONSULTANT accordingly. 5. Except as modified by this Amendment, all other terms and conditions of the Agreement remain the same. ATP's main focus is service and repair of fitness equipment, having its primary place of business located at 4067 Hardwick Street, Suite 495, Lakewood, CA 90712. ATP will provide maintenance and service on fitness equipment for the customer. ATP agrees to prWde the lullowing services to rnaintain pa uducts listed on a basis. This included gig -hour response to the customer facility, as needed, for repairs. Any additional visits will be ratedper the agreement, excluding parts (see Schedule A). This agreement is a flat rate amount supporting the customer. The customer agrees to pay the sum of $J61LIper visit for services- listed and payment is due 30 days from the completed service date. This will include: Overall inspection, adjustments, calibration, lubrication, minor on -site repairs (as needed), external detailing and detailed summary reports of work performed. This agreement becomes active on the first date of service Parts, labor and any additional charges will be quoted prior to any repairs being made. In the event this contract proves unsatisfactory to either party, it may be terraainated by a thirty (30) day written ('100ce by eitiaea' party, Payment terms: Net thirty (30) days from the date of service.
